Root Cause Analysis in Healthcare

Root cause analysis (RCA) originated in engineering, but is now routinely used in healthcare to investigate patient safety incidents and facilitate organisational learning. Aimed at both clinicians and non-clinicians at any level, our RCA course focuses on theory and application of RCA techniques.

Aims and Objectives

The aim of this course is to enable clinicians to carry out RCA investigations and understand how learning from them can be cascaded to improve practice.

Intended Learning Outcome

  • Increase understanding of the theory and application of RCA.
  • Gain insight into the skills required for effective RCA.
  • Be able to carry out RCA using the tools and techniques demonstrated to investigate an incident.
